FINDINGS AND ORDER AFTER HEARING
THIS CAUSE having come before the Court upon Defendant’s Objection to Plaintiff’s Supplemental request to Produce and Interrogatories, and the Court having heard argument of counsel, and being otherwise advised in the Premises, it is hereupon,
ORDERED AND ADJUDGED that said Objections be, and the same is hereby overruled in part and sustained in part as follows. DEFENDANT shall produce a report including every claim from medical providers in Geozip 334 for the time period from three (3) months prior to, and three (3) months subsequent to the dates of service August 22, 2011 through August 29, 2011 for the following CPT Codes involved in the current litigation, 97001, 97110, and 97140. Said Geozip report shall include the claim number, the date of service, the CPT code, the amount of each healthcare providers’ charge for each CPT code, the name of the healthcare provider and the zip code of each healthcare provider.
